Ohio requires a registered sewage treatment system service provider — who services, maintains, and operates STS — to maintain a $25,000 statewide bond under ORC Chapter 3718. Ours is $750 flat — 3% of the bond amount, the same for every provider. No credit check.

Registration bonds are the simplest thing in surety. Here's the entire process:
Business details and an effective date. That's the application — no financials, no credit check section.
Registration bonds like this are among the thousands that issue right after purchase. At most, 1–2 business days.
The original executed bond is submitted to ODH for your STS service provider registration. Wet-ink original mailed on request.
$25,000 bond × 3% = $750, one-time per term. Fixed amount, fixed price, multi-year if you want it.
Ohio registers sewage treatment system service providers under ORC Chapter 3718 and OAC 3701-29-03 — the people who service, maintain, and operate STS, including the operation-and-maintenance work that many systems require. The registration is conditioned on a statewide surety bond filed with the Department of Health.
The standard service provider bond is $25,000. A provider who also holds an STS installer registration may carry a reduced $15,000 service provider bond, because the installer bond already provides part of the financial security.
The bond provides financial security for enforcement of the registration law and protects against improper service work. Like the installer and hauler bonds, this bonding is statewide — local health districts may not require an additional service provider bond.
These are the actual issuing fields — no credit check section, because this bond doesn't have one.
