Solicitation No. 75H70726Q00107 is issued as a Request for Quotation (RFQ) for one (1) Nurse Practitioner to provide non-personal outpatient primary care services for the Ute Mountain Ute Health Center (UMUHC) located in Towaoc, Colorado.
This acquisition is set aside for Indian Small Business Economic Enterprise (ISBEE) concerns. This solicitation incorporates provisions and clauses by reference and in full text. The full text of FAR provisions and clauses may be accessed electronically at www.acquisition.gov. Applicable agency deviations are identified in the attached RFQ.

The period of performance consists of a one-year base period and four (4) one-year option periods. See attached RFQ for anticipated performance dates and inspection and acceptance requirements.

(viii) This is a 100 percent Indian Small Business Economic Enterprise (ISBEE) set-aside under the Buy Indian Act. The applicable NAICS code is 621399, with a small business size standard of $10 million.
The Contractor shall provide a Nurse Practitioner to deliver non-personal outpatient primary care services at the Ute Mountain Ute Health Center (UMUHC) located in Towaoc, Colorado. Services include comprehensive primary care, including patient assessment, diagnosis, treatment, health maintenance, and preventive services for patients of all ages. The Contractor must coordinate care with physicians, nursing staff, and other health professionals, maintain medical records, and comply with all applicable Federal, State, and IHS policies and procedures.
