This amendment provides responses to vendor questions, clarifies BLS, ACLS, and PALS certification requirements, and extends the quotation deadline to Monday, November 9, 2026, at 5:00 p.m. MST.

This is a combined synopsis/solicitation for commercial services prepared in accordance with FAR Part 12. This announcement constitutes the only solicitation. Quotes are being requested and a separate written solicitation will not be issued.
Solicitation No. 75H70726Q00107 is issued as a Request for Quotation (RFQ) for one (1) Nurse Practitioner to provide non-personal outpatient primary care services for the Ute Mountain Ute Health Center (UMUHC) located in Towaoc, Colorado.
This acquisition is set aside for Indian Small Business Economic Enterprise (ISBEE) concerns. This solicitation incorporates provisions and clauses by reference and in full text. The full text of FAR provisions and clauses may be accessed electronically at www.acquisition.gov. Applicable agency deviations are identified in the attached RFQ.
(ii) See attached RFQ and Fillable Pricing Table for Contract Line Items (CLINs), quantities, units of measure, and option periods.
(iii) See attached RFQ for the description of requirements. Instructions to Quoters and Evaluation are provided in Section F.
(iv) Services shall be provided at the following Albuquerque Area Indian Health Service (IHS) facility:

The period of performance consists of a one-year base period and four (4) one-year option periods. See attached RFQ for anticipated performance dates and inspection and acceptance requirements.

(viii) This is a 100 percent Indian Small Business Economic Enterprise (ISBEE) set-aside under the Buy Indian Act. The applicable NAICS code is 621399, with a small business size standard of $10 million.
The Nurse Practitioner will provide non-personal outpatient primary care services for the Ute Mountain Ute Health Center (UMUHC) located in Towaoc, Colorado. Services include family medicine, internal medicine, pediatrics, geriatrics, and limited x-ray services. The contractor shall provide comprehensive primary care, including patient assessment, diagnosis, treatment, health maintenance, and preventive services. The contractor must coordinate care with physicians, nursing staff, behavioral health providers, pharmacists, and other interdisciplinary health professionals. The contractor shall participate in scheduled clinics, same-day appointments, walk-in patients, and quality assurance activities. The contractor must comply with applicable Federal, State, and local laws, IHS policies, and clinical standards.
