A contractor who does plumbing work, or only installs appliances into the plumbing system, anywhere in Darke County, Ohio must register with the Darke County General Health District, and the registration requires a $20,000 surety bond kept in force for the year. The Health District will not register a plumbing contractor or appliance installer until the $20,000 bond is on file with the registration application. Here is the whole process, from application to registered:

The Darke County General Health District runs a Plumbing Inspection Program that registers every person engaged in the business of plumbing in the county. Its posted registration applications — one for plumbing contractors, one for appliance installers — each carry the same condition: the registrant agrees to maintain a $20,000 bond, and if the surety bond is canceled, the registrant must immediately submit proof of a new registration bond. A contractor who only installs appliances registers as an Appliance Installer rather than a full plumbing contractor, but files the same $20,000 bond.
The bond is written on the Health District's own form, titled Bond for Plumbing Contractors/Appliance Installers and Plumbing Employees. It binds the principal and surety to the Darke County Department of Health for a twelve-month term ending December 31, conditioned on the principal conforming to Ohio law and to the Health District's plumbing rules and regulations, and holding the Health District harmless. The form notes that contractors, business firms, and self-employed installers must post the bond; employees of a bonded contractor do not need their own.
It is a three-party arrangement: you (the principal), the surety carrier, and the Health District (the obligee). It is not insurance for you — if the surety pays a claim because your work broke the rules, you repay the surety. Registration also does not replace job permits: an appliance installer registration covers appliances only, and the Health District still requires a plumbing permit for work such as water heater installation and replacement.
