CommUnityCare manages referrals to specialists across Travis County and surrounding areas to ensure timely patient access to care. We seek a proven third-party vendor that can provide a closedloop referral management solution capable of tracking referrals from creation through final resolution, including scheduling, completion, consultation receipt, and communication back to the originating provider.
The vendor shall provide a comprehensive referral management platform supporting the complete lifecycle of outbound specialty referrals and diagnostic imaging facilitation, including referral intake, authorization management, specialist scheduling status monitoring, consultation retrieval, referral closure, analytics, and interoperability with Epic. The platform must support electronic receipt of referrals with Epic integration, duplicate referral detection and prevention, prior authorization management, closed-loop referral management with status monitoring and alerts, specialist consult note retrieval with PCP notification, bi-directional communication, and optional diagnostic imaging facilitation and tracking. The vendor must also provide analytics and dashboards, patient engagement capabilities (SMS reminders, automated patient outreach, multilingual communications), and comply with HIPAA and security requirements. The vendor shall guarantee 99.9% system availability, maintain referral and interface data accuracy of at least 99.5%, notify CommUnityCare of security incidents within 24 hours, and achieve support and resolution times as defined in the Service Level Agreement.
