The vendor must print 250,000 copies of the CHJ 549 Health Care Request Form (rev 5-19) at a finish size of 8 1/2 x 11 upright, using 3 part NCR (White/Canary/Pink) with an NCR pad at top. Part 1 & 2 print the same, Part 3 prints different, and Part 2 ONLY is microperforated across the 8 1/2" dimension 5/16" down from the top. An electronic file is provided, a PDF sample is attached to the bid, and a PDF proof is required for approval. Packing is shrink wrap 100 sets + 1 chipboard/package, 10 packages/1000 copies per carton, on 4 way skids, and the forms must be packed in plain unmarked cartons with only form number and quantity on the box. The job ships to Jackson, MI 49201, and the vendor must attach a detailed quote letter(s).
