The contractor must provide a DxH 690T Hematology Machine on a lease, including all control material, reagents and supplies on a metered reagent plan, installation, laser printer, CPU, modem, touch screen LCD flat panel monitor, uninterruptible power supply, operator maintenance and chemistry/diagnostic manuals, and a second UPS. The instrument must be able to calibrate and QC multiple lots of reagents simultaneously, calibrate and run specimens at the same time, have little to no daily maintenance, retain calibration information upon reboot, include electronic maintenance logs, a clot detection mechanism, automated maintenance protocols, an online operator's manual, alert staff when maintenance is due, run multiple tube types, include all linearity material for startup, provide 2 training slots (including meals, airfare, and lodging or training material), have a stat load position while running, include instrument, reagents, maintenance, calibrators, and controls, include all preventive maintenance, and the company must perform all parts (including shipping) and labor for the instrument to perform properly. All consumables must be included in the metering price. The system must accept pre-assigned work lists from LIS, transmit numerical sample results, flags, graphics, and QC results to host computers, have bi-directional communication capability, have 2 identification fields for inmate name and number, and property tax is the vendor's responsibility. Monthly service includes preventive maintenance with all parts, labor, and travel on site, Monday through Friday coverage if the analyzer is down, emergency service calls with all parts, labor, and travel, on-site support within 24 hours of notification, 24-hour technical phone support, and modifications at no additional cost. Lines 3-10 cover hematology reagents, controls, and consumables compatible with an automated hematology analyzer, with estimated quantities determined by doctor-ordered tests.
