Services include case management (weekly face-to-face contact, crisis intervention, coordination of resources), life skills planning and development (individual assessments, transition plans, monthly transitional workshops, service navigation, food allotment of $150 every 30 days, transportation support), and health and well-being coordination (building permanent connections, accessing physical/mental/behavioral health services, services for pregnant/parenting young adults, youth advocacy opportunities). The Contractor must maintain specified staffing ratios (SIL Program Manager 0.5 FTE per 8 young adults, Program Coordinators 0.5 FTE per 8 young adults, additional staff supports 0.25 FTE per 8 young adults) and submit quarterly financial reports.
