The selected A/E firm will perform existing conditions and programming, concept design, cost estimating, design development, construction procurement, construction administration, and closeout for a two-phase remodel of the Siletz Community Health Clinic. The remodel covers approximately 7,655 square feet across zones including Conference/Training Room 147, X-Ray/Technician Suite 163-164, Business Office 256/Storage 207, Optometry Reception/Waiting, Conference 245/Shell Space 244, and Pharmacy/Lobby/Front Desk. Services include site visits, staff programming, concept design, cost estimates, design development through 65%, 95%, and final milestones, preparation of construction RFP/ITB, submittal review, RFI response, site observations, pay application review, and closeout documentation. The project must maintain clinic operations during construction, with a two-phase construction plan to be finalized during design.
