The New Hampshire Department of Health and Human Services, Division of Public Health (“Department”) is seeking responses to this Request for Proposals ("Solicitation" or "RFP") from qualified Vendors to support the Department’s chronic disease, screening, and
preventative health programs. The selected Vendor will provide support across program areas by coordinating partner engagement, assisting with evidence-based intervention activities, and supporting evaluation, training, administrative, and communication functions as directed by the Department.
The Contractor must support the Department's Chronic Disease Prevention and Wellness Programs by coordinating and implementing Innovative Pilot Projects in collaboration with public health entities, healthcare organizations, community-based organizations, and other approved stakeholders. Services include supporting implementation of strategic program priorities, facilitating collaboration among partners, assisting with coordination of evidence-based interventions, providing evaluation and training support, and supporting administrative and communication activities. The Contractor must provide a staffing plan including Program Leads, a Project Director/Administrator, Training Coordinator, Communications Specialist, and Evaluation & Data Specialist. The Contractor must also administer Innovative Pilot Projects, coordinate continuing education credits, participate in monthly meetings, and submit monthly reports.
