The provider must deliver a full continuum of services including active treatment, behavioral and psychiatric support, nursing and medical oversight, habilitation programming, crisis intervention, and comprehensive discharge planning for up to 20 individuals. The facility must be a secure campus-style environment with 24/7 awake staff, and the provider must obtain and maintain ICF/IID Medicaid certification. Services include room and board, nursing care, medical supplies, durable medical equipment, mental health services, therapy services, transportation, and habilitation services, all covered by a daily per diem rate. The provider must also manage admissions, develop person-centered discharge plans within 90 days, and comply with all CMS, IDOH, and BDS regulatory requirements.
