The vendor shall provide necessary staff and facilities to complete Pre-Employment Physicals with Weightlifting Assessment & Respirator Medical Questionnaire, Drug Screenings (10 panel testing from a urine sample), and Pulmonary Function Tests when an applicant fails the Respiratory Medical Questionnaire. Physical examinations shall be completed to the most recent standard, lift tests shall be completed in intervals of 25 and 50 pounds 3 repetitions each, and evaluation and documentation shall meet OSHA Respiratory Protection Standard 29 CFR 1910.134. All services shall be rendered at the vendor's medical facility within a 20-mile radius of the facility. All results shall be delivered to the facility within 72 hours of completion of testing.
