The VA North Texas Health Care system requires community based Outpatient clinic (CBOC))

The contractor shall provide comprehensive primary care services using the Patient Aligned Care Team (PACT) model, including but not limited to: preventive screenings, diagnosis and management of acute and chronic conditions, women's health services (including breast and cervical cancer screening, contraception, maternity care), mental health services (including screening, suicide prevention, and substance use disorder care), pharmacy services (including prescription fulfillment and medication management), ancillary support services (radiology, laboratory, point-of-care testing), telehealth services, and coordination of specialty care referrals. The contractor must staff the clinic with a Physician Director, Primary Care Providers (Physicians, APRNs, or PAs), RN Care Managers, Clinical Associates, and Administrative Associates in specified ratios. The contractor shall provide all necessary space, equipment, and supplies, and comply with all VA policies, performance standards, and accreditation requirements.
