Evident Change will provide technical assistance, consultation, and training support to CYFD for the ongoing implementation of the Safety Organized Practice (SOP) Model and the Structured Decision Making (SDM) System. The work includes finalizing the implementation of the Statewide Central Intake (SCI) SDM intake assessment, supporting the Family Outreach and Community Engagement (FORCE) Program, increasing CYFD's overall capacity in SOP and the SDM system, and creating a reunification assessment. Specific deliverables include developing a continuous quality improvement (CQI) system, holding bi-annual discussions on progress metrics, providing SCI coaching, conducting SDM intake revisions, delivering onsite workshops and remote coaching, producing an SDM management report, designing an initial reunification assessment, developing reunification assessment curriculum, delivering training, and deploying the reunification assessment in the SDM data collection system.
