The New Hampshire Department of Health and Human Services, Division of Medicaid Services(“Department”) is seeking responses to this Request for Proposals ("Solicitation" or "RFP") from qualified Vendors to establish a statewide Medicaid Medication Risk Reduction Initiative (MMRRI) led by New Hampshire licensed clinical pharmacists to reduce the prevalence of, and prevent, Medication-Related Problems (“MRPs”), Adverse Drug Events (“ADEs”), polypharmacy, over-prescribing, behavioral health related medication risks, and opioid related medication risks of New Hampshire Medicaid beneficiaries.
The selected Contractor shall employ or contract with New Hampshire licensed advanced clinical pharmacists to create a center of Community Pharmacists who shall conduct Comprehensive Medication Reviews (CMRs) of Medicaid beneficiaries identified as meeting polypharmacy or over-prescribing criteria or at high-risk of a Medication-Related Problem (MRP) or Adverse Drug Event (ADE), including those prescribed opioids or behavioral health medications. Contemporaneous to conducting the CMR, the Community Pharmacists shall generate Medication Safety Reports (MSRs) with actionable steps for the patient and their medical provider. The Contractor must provide a technology solution to analyze Medicaid pharmacy encounters and claims data to create comprehensive medication lists, identify beneficiaries at risk, and support the pharmacists in making recommendations. The Contractor must also partner with medical providers, MCOs, and the Department to communicate MSRs, develop provider incentive strategies, train providers, and launch community-based programs to promote safe medication use. The Contractor must ensure services are available statewide with a focus on rural areas, and must guarantee a minimum per-member-per-month (PMPM) cost reduction in the Medicaid Managed Care program by the end of Contract Year 3.
