The contractor shall provide optometry services as requested to DJJ residents at the facility location (on-site) including, but not limited to: comprehensive eye examinations for residents referred by medical staff, including inspection of sclera, lids, conjunctiva, cornea, and eye positions; laboratory services to fill prescriptions for glasses, replace broken lenses, and adjust and repair frames as required; provide requested services only when a minimum of four (4) juveniles require care or at the request of the facility; provide a rate structure detailing daily cost of providing a clinic of minimally four juveniles and daily cost to service juveniles above the 4 limit; permit juveniles to select any style that does not exceed scheduled price; bill DJJ for only those costs approved and outlined on the fee schedule; provide contact lenses only when improvement in vision would be substantially greater than that achieved from glasses; supply DJJ a copy of the full examination to place in the juveniles medical record; abide by all HIPAA policies; provide all equipment and products necessary; participate in Quality Assurance efforts; comply with National Commission on Correctional Health Care standards; designate another duly licensed optometrist to serve in the contractor’s absence; and adhere to DJJ policy and procedure relative to medical emergencies.
