experiencing homelessness find and sustain stable, safe, permanent housing.
Agencies will do this by implementing interdisciplinary service teams that address
the individualized needs of each consumer. The service teams will follow the
consumer from their unsheltered or sheltered situation through the stabilization of
permanent housing. The program will follow a “services follows the person” model.
