Medical Respite Transitional Housing

The contractor provides 24-hour transitional housing with beds, furnishings, linens and lockable storage, three nutritionally adequate meals daily plus snacks, laundry, hygiene supplies, and secure medication storage and monitoring. The contractor delivers psychosocial assessments and person-centered care plans within 5 business days, weekly case management, structured therapeutic group activities, substance-use support with drug testing, transportation to medical, housing, employment and court appointments, and assistance with income, benefits, budgeting, permanent housing, and coordination of medical and mental health care, while operating a compliant facility with 24-hour awake supervision, medical and case-management staffing, documentation, incident reporting, and quality monitoring.
