The contractor shall provide elevator inspection services under a 12-month base contract period, including four option years. Services shall include the performance of a Five-Year Safety Test (Category 5) inspection as well as annual and semiannual Safety Tests inspections (Category 1) for elevators and dumbwaiters. All services shall be performed at the Brooklyn VA Medical Center, 800 Poly Place, Brooklyn, NY 11209-7104 and St. Albans VA Medical Center, 179-00 Linden Boulevard, Queens, NY 11424-1468. The contractor will witness and certify the inspections, document code violations, and submit reports. The contractor shall also perform re-inspections of corrective actions if an elevator or dumbwaiter fails an inspection.
