KIOSKS, BLOOD PRESSURE (RENTAL)
The vendor must provide blood pressure kiosks conforming to detailed specifications (Class II medical device, oscillometric measurement, weight scale, oximeter, barcode reader, magnetic stripe reader, touch screen LCD display, etc.) on a rental basis. The vendor must deliver, install, and place the kiosks in operation at designated locations, provide training, and perform all preventative and remedial maintenance at no additional cost. The vendor must maintain and repair damaged kiosks and replace units lost by theft. The contract is for three years from date of award with two optional one-year extensions. Pricing must be firm for the first year, with potential price increases limited to 5% per year and requiring 45 days written notice. The vendor must provide quarterly reports detailing purchases. Delivery must be F.O.B. destination, and the vendor must provide 24/7 emergency contact and 2-hour emergency parts availability.
