Utah conditions a medical cannabis pharmacy license on a $100,000 performance bond issued by a surety authorized to transact surety business in the state — Utah Code 4-41a-1001(2)(b)(iii), filed with the Department of Agriculture and Food. Pricing is 2% of the bond amount, $100 minimum, your exact price appears at the application, and the bond issues the moment you pay. Any credit screen is a soft pull only — it never affects your score.

No underwriting queue for the pharmacy performance bond — enter your amount, pay, and file with the Department. Here is the whole thing:
Business details, the bond amount your license requires, an effective date, and a term. Any credit screen is a soft pull that never shows as a hard inquiry.
The premium is 2% of the bond amount, $100 minimum — priced at checkout, so the bond issues the moment you pay. Your executed bond and power of attorney generate on the spot.
Your executed bond arrives by email, ready to submit with your medical cannabis pharmacy application to the Utah Department of Agriculture and Food. Wet-ink original mailed on request.
Utah licenses medical cannabis pharmacies under the Utah Agricultural Code, Title 4, Chapter 41a. Since H.B. 72 moved pharmacy and courier oversight from the Department of Health and Human Services on July 1, 2023, the licensing agency is the Utah Department of Agriculture and Food, working with the licensing advisory board created under Section 4-41a-201.1.
An applicant must obtain and maintain a performance bond in the amount of $100,000, issued by a surety authorized to transact surety business in the state, for each application submitted to the Department. The Department may not issue the license until the selected applicant has the bond in place — so the bond is a gating item, not a post-award formality.
It is a three-party arrangement: you (the principal), the surety carrier, and the State of Utah (the obligee). The bond stands behind your compliance with the chapter and the rules made under it — and it is not insurance for you. If the surety pays, you repay the surety. The bond has to stay continuously in force for as long as you hold the license; we track the term and send renewal notices 60 and 30 days out.
These are the actual issuing fields — business details, the bond amount your license requires, an effective date, and a term. That is the entire application.
