HHSC PCS is seeking competitive bids to establish contract(s)for concrete services for Rio Grande State Center.
People whorequire ADA accommodations and wish to attend or participate in the pre-bidmeetings and require auxiliary aids or services should contact the Sole Pointof Contact identified in Section 6.1(Sole Point of Contact and Communications), at least seventy-two (72) hoursbefore the meeting in order to have reasonable accommodations made by HHSC.
Attendees, inperson, are required to sign an attendance log at the conference or on-sitevisit. HHSC will not be held responsible for an attendee’s failure to sign inon the attendance log.
For conferencecalls and webinars, participants will be added to the attendance log and shouldhave the following information ready and properly announced or submitted:
a) Legalbusiness entity name or name of individual which will be used if submitting aresponse to this IFB or subcontractor acting on behalf of the legal businessentity;
b) Name andtitle of each representative on the call or webinar; and
c) E-mailaddress for the point of contact.
HHSC will conduct mandatoryon-site visit at the location specified below on the date and timeidentified in the Procurement Schedule (cover page of this IFB) or on the dateand time established in subsequent Addenda.
The on-site visit will include a review of the IFBrequirements as well as a walk-through of the facility.
Attendance atone site visit is mandatory. Failure to attend in person will result in thedisqualification of a Bidder. Bidders not listed on the attendance log will notbe permitted to submit a Response.

The required services are the complete removal and replacement of damaged sidewalks at Rio Grande State Center, including breaking up old concrete, hauling away debris, preparing and compacting subgrade, setting forms, pouring new concrete to proper grade and slope, and applying a clean broom finish with necessary expansion joints. Work is specified at Building 503 (four sections totaling 631 sq ft), Building 505 (two sections totaling 186 sq ft), and Building 510 (two sections totaling 186 sq ft). The contractor must assure all underground utilities are marked, sawcut concrete a minimum of 3 inches through the joint, avoid damage to adjacent slabs and structures, compact subgrade to 95% Proctor density, use #3 rebar at 16" O.C., pour 3000 PSI concrete at 4-6" thickness, and meet all ADA requirements.
