The contractor must provide orthopedic shoes and orthotics as specified in the Pricing Sheet (Exhibit C), with delivery to the Richmond State Supported Living Center. Deliveries must be made between 8:00 AM and 4:00 PM on the specified date, with a delivery appointment required. Products must be labeled on the outer carton with description of contents, agency purchase order number, and quantity per carton. Pallets must be shrink wrapped and labeled. The contractor is responsible for all labor, tools, transportation, equipment, materials, and supplies necessary to provide the goods.
