Orientation & Mobility Services needed for RSSLC

The contractor will work with patients who have vision impairments to achieve safe travel in any environment as independently as possible; provide training to staff in safety transporting and escorting individuals with visual impairments; conduct evaluations of patients per facility policy utilizing various assessment instruments and techniques; work collaboratively with multi-disciplinary team; attend individual support plan and addendum meetings; complete and submit service reports within facility timelines; adhere to the practice rules and regulations of Academy for Certification of Vision Rehabilitation & Education Professionals (ACVREP); comply with facility policy; implement services and supports in a timely manner per the treatment plan; complete and submit required service reports timely; attend requested meetings and trainings; modify treatment plan when individual presents no improvement; and commit to providing service at minimum of 1 day (8 hrs. total) per week between 8am-5pm.
