Stony Brook University Hospital intends to purchase Mini Stick Max & and related items, manufactured by Angiodynamics under its discretionary authority, pursuant to NYS Education Law §355(5)(A), which is up to $125,000, for the above noted commodity/product or service. Failure to comply with this requirement can range from no contract to debarment from sales to any NYS Agency. Successful contractor shall be required to complete and submit documents related to Vendor Responsibility, NYS Finance Law 139, NYS Tax Law Section 5-a, and valid/current NYS Workers’ Compensation and Disability Forms.
