The contractor will perform an Initial Priority Assessment using aggregated provider-level data to identify underperforming FQHCs, identify champions within health systems, conduct stakeholder needs assessments, develop training plans and materials, evaluate technical assistance, build capacity with non-participating FQHCs, and provide ongoing communication and reporting including monthly reports, bi-monthly meetings with MS-BCCP staff, and quarterly progress reports.
