The vendor must provide quantitative fit testing for 99 Avon Protection C50/FM54 Full Face Masks, 99 N95 facemasks, and 99 3M 6000 Series Half Masks. The fit testing facility must be 10 to 11 miles from the department address at 1104 N. Mission Rd, Los Angeles, CA 90033. The vendor must accommodate 99 employees within 1 year, scheduling 1 to 4 employees per business day, Monday through Friday 8:30am to 5:00pm. Fit testing must be performed using TSI PortaCount Equipment in accordance with California OSHA Title 8:5144 appendix A. The vendor must provide a certification card or proof of pass/fail test results within 48 hours following testing. The vendor must be able to conduct 3 fit tests per hour, per qualified fit test staff, and provide a flat rate per fit test.
