RFB - Request For Bid
The awarded vendor must supply plastic library cards as specified in Exhibit A. Line Item 1 covers 263,500 Child and Adult Library Cards - Plastic, and Line Item 2 covers 5,000 Custom Library Cards - Plastic, Special Event with custom design based on artwork provided by the department. Cards must be shipped within 30 days after receipt of order. All bidding vendors must submit sample library cards with imprinted bar codes at no charge. The bar code must be in CODABAR format with an eye-readable number in OCR-A format. The cards must be made of PVC, finished size 4.5" x 2.125", final thickness .030", core color white, with specific printing and design specifications. The contract has an initial term of three years, renewable for up to two additional one-year periods.
