RFP-57-27 KASTOW - ORPHAN WELL PACKAGE - BREATHITT #002

The contractor shall properly plug, site remediate, and reclaim orphan wells identified in the package in compliance with KRS Chapter 353.562-564 and 805 KAR 1:060. Each well will be plugged per the attached plugging plan (Attachment K) under Division inspector direction. The contractor shall remove associated aboveground flowlines and production infrastructure at the wellsite. Disturbed areas must be reclaimed by implementing best management practices to achieve approved vegetative cover and proper site drainage with erosion control. Sites must exhibit permanent vegetative cover with effective drainage for one year before final acceptance and payment. If tanks are present, laboratory testing for TENORM is required for tank bottom sludge materials, with a typical 30-day analysis turnaround; all metal must be scanned for TENORM. Disposal of TENORM material requires chain-of-custody and an approved disposal site; contractor shall submit invoice for cost plus 10% for administration. Estimated quantities of oil/brine contaminated soil and tank bottom sludge must be removed to an approved landfill. After plugging, wells must be cut off three feet below surface, all wellhead equipment removed, and the site reclaimed.
