One of the goals of PEPFAR in Mozambique is to support efforts to achieve HIV epidemic control through implementation of evidence-based interventions to drive progress in HIV prevention, care and treatment and ultimately, save lives</p> <p><span style="color: black;">&nbsp;</span></p> <p>Through the P<span style="color: black;">EPFAR Coordination </span>Office<span style="color: black;">, the U.S. Embassy in Maputo administers the </span></p> <p><span style="color: black;">Community Grants Program (formerly Small Grants)</span> <span style="color: black;">and is seeking proposals from registered</span></p> <p><span style="color: black;">community-based organizations whose mission and activities focuses on HIV programming.</span></p> <p><span style="color: black;">&nbsp;</span></p> <p><strong style="color: black;">Objectives</strong></p> <p><span style="color: black;">&nbsp;</span></p> <p><span style="color: black;">As PEPFAR continues to confront challenges with</span> retaining HIV+ individuals in care and treatment programs, our <span style="color: black;">collaboration with communities and patients is urgent and critical. Collaborating with community members that will identify barriers and enablers to accessing and utilizing HIV services will be key in im</span>proving retention in Mozambique, as will ensuring stigma and discrimination surrounding HIV is addressed. Organizations will be expected to carry out patient surveys and gather information on the patient experience at health facilities. This information will then be used to facilitate dialogue and advocacy for improvement. Training and technical assistance will be provided to organizations selected for this award.</p> <p><span style="color: black;">&nbsp;</span></p> <p><span style="color: black;">This NOFO </span>seeks<span style="color: black;"> eligible organizations </span>to<span style="color: black;"> train, support, equip and </span>compensate PLHIV to carry out routine monitoring of the quality of HIV treatment services<span style="color: black;"> </span>received by patients at health facilities. We are looking for organizations that can help communities directly impacted by HIV to facilitate dialogue, based on patient experiences, that will help drive changes and improve HIV services in Mozambique.</p> <p>&nbsp;</p> <p><span style="color: black;">This mechanism will help PEPFAR</span> <span style="color: black;">programs, Public Health facilities and Local Government</span></p> <p><span style="color: black;">Institutions </span>to gain insights on <span style="color: black;">persistent problems</span> impacting the quality of care of HIV and identify s<span style="color: black;">olutions or enablers </span>that will<span style="color: black;"> ensure that beneficiaries receive quality services and will encourage them to retain in care and adhere to their HIV treatment.&nbsp;</span></p> <p>&nbsp;</p> <p><span style="color: black;">Additional details on potential activities under these areas are provided in Section 4.</span></p> <p><br></p> <p><strong style="color: black;">1.&nbsp;&nbsp;&nbsp;&nbsp;FEDERAL AWARD APPLICATION INFORMATION</strong></p> <p><span style="color: black;">For all application documents, please ensure that all documents are in English and all costs are in U.S dollars. </span></p> <p><span style="color: black;">&nbsp;</span></p> <p><span style="color: black;">1.1&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Applicants should submit ONE project proposal that does not exceed TEN pages </span></p> <p><span style="color: black;">1.2&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;All documents are 1.5 spacing, 12-point Times New Roman font </span></p> <p><span style="color: black;">1.3&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;All pages are numbered including budgets and attachments</span></p> <p><span style="color: black;">1.4&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Each proposal MUST include also a one-page summary that includes </span>an Executive summary, outlining project goals, objectives, proposed cost and activities.</p> <p>&nbsp;</p> <p><strong style="color: black;">2.&nbsp;&nbsp;&nbsp;&nbsp;APPLICATION REQUIREMENTS</strong></p> <p>Complete application MUST include the following:</p> <p>&nbsp;</p> <p><span style="color: black;">2.1&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Completed and signed SF-424</span> <span style="color: black;">and SF-424B forms </span></p> <p><span style="color: black;">2.2&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Cover Page (not exceeding ONE page, word document) including a table with organization name, project title, target districts/location, name and contact information of the </span>organization’s primary point of contact</p> <p><span style="color: black;">2.3&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Table of Contents (not exceeding ONE page) listing all documents and </span>attachments<span style="color: black;"> with numbers</span></p> <p><span style="color: black;">2.4&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Proposal Narrative (not exceeding </span>FIVE<span style="color: black;"> pages). Grantees will be instructed on the inputs required for financial and programmatic progress reports by the PEPFAR Coordination Office.</p> <p>&nbsp;</p> <p><strong style="color: black;">5.&nbsp;&nbsp;&nbsp;&nbsp;DEPARTMENT OF STATE FUNDING RESTRICTIONS</strong></p> <p><span style="color: black;">Per Department of State policy, funding cannot be used for the following purposes: </span></p> <p><span style="color: black;">●&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Construction of buildings; </span></p> <p><span style="color: black;">●&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Salary, rent, utilities or office furniture related to the organization applying for this funding; </span></p> <p><span style="color: black;">●&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Land purchase, security guards, motor vehicles (including motorcycles, but excluding bicycles), car insurance, motor vehicle repairs or upkeep; </span></p> <p><span style="color: black;">●&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Outside audits; </span></p> <p><span style="color: black;">●&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Purchase of alcoholic beverages; and</span></p> <p><span style="color: black;">●&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;Activities contrary to the Protecting Life in Global Health Assistance Policy.</span></p> <p><strong>&nbsp;</strong></p> <p><strong style="color: black;">6.&nbsp;&nbsp;&nbsp;&nbsp;FUNDING ALLOCATION</strong></p> <p>Funding will be limited to a maximum of $30,0000 per implementation period for each project (minimum period of implementation is for six month).&nbsp;Funding is expected to be allocated in the districts listed below; applicants may only send a proposal focused on one of the chosen geographic area.
