DFS-DNA Buccal Swab Kits - tt

The vendor will manufacture, stock, and deliver DNA Buccal Swab Collection Kits as needed. Kits must include specific components such as a pre-printed return envelope, a kit/form, swab envelope, three wooden-shaft cotton sterile swabs, a kit shipping seal, fingerprint sheets, fingerprint re-tab strips, biohazard barrier gloves, four bar code labels, an FDA insert, and shipping containers with a notification of changes insert. The vendor must submit proof copies within 30 days of the last contract signature, have kits available within 45 days of the initial order, and deliver within 30 calendar days after subsequent orders. Weekly electronic kit inventory reports must be provided to the DNA Indexing Laboratory Director or Assistant Director.
