The VA North Texas Health Care system requires community based Outpatient clinic (CBOC))

The contractor shall provide comprehensive primary care services using the Patient Aligned Care Team (PACT) model, including diagnosis and management of acute and chronic conditions, preventive care, women's health services, mental health screening and care, pharmacy services, ancillary support services (radiology, laboratory, point-of-care testing), telehealth services, and coordination of specialty care referrals. The contractor must staff the clinic with a Physician Director, Primary Care Providers (physicians, APRNs, or PAs), Registered Nurse Care Managers, Clinical Associates, Administrative Associates, and ancillary staff (X-Ray technologist, phlebotomist). The contractor shall provide space, equipment, and supplies, and comply with all VA policies, performance standards, and quality improvement requirements. The place of performance is within the city limits of Sherman, Texas.
