The contractor shall provide all personnel, equipment, supplies, transportation, and supervision necessary to perform dental amalgam waste recycling services, including provision of 5-gallon amalgam recycling kits for collecting contact and non-contact amalgam, vacuum traps, chairside traps, extracted teeth with amalgam, capsule waste, and scrap amalgam; provision of a DU10 amalgam separator replacement kit; pre-paid shipping to and from clinics; recycling at a USEPA-certified facility; and a complete cradle-to-grave audit trail with certificates of recycling.
