Family Assistance Program Alcona Iosco Counties

The contractor will provide Family Assistance Program services including modeling and demonstrations in child nurturing, home management, nutrition, transportation, support attending hearings and medical appointments, and support utilizing community resources and Infant Safe Sleep practices. The contractor will also provide Parenting Time support to reduce the risk of child abuse and/or neglect and out-of-home placement. Services include face-to-face contacts in client homes, developing service plans, providing transportation, accompanying clients to appointments, and documenting progress. The contractor must meet with clients within seven calendar days of referral, provide at least four face-to-face contacts per month, and submit various reports to MDHHS.
