RFP-2909 SOLICITATION OF COUNTY INTENSIVE CASE MANAGEMENT SERVICES

1. County of Chemung Department of Social Services (DSS) seeks proposals from qualified community agencies (“the Contractor”) to provide two full‑time caseworkers who will deliver intensive family or person-centered case management services focused on self-sufficiency, housing stability, and reduced reliance on Public Assistance.

All referrals will be made exclusively by DSS. Referred individuals and families will be those who are currently receiving Public Assistance or applying for Public Assistance due to housing instability, income loss, or related barriers.

The purpose of the program is to reduce or prevent ongoing reliance on Public Assistance by stabilizing housing, supporting income growth, and addressing barriers through strength‑based, trauma‑informed, and innovative practices. The program is designed to help households achieve and maintain stability so that Public Assistance is not needed long‑term.

2. It is the intent of the County to select a qualified contractor(s) to provide timely, cost-effective intensive case management services.
