The contractor must provide an evidence-based, family-centered childhood obesity intervention program using a CDC-approved curriculum. The program must enroll a minimum of 25 children and their families, build structured referral pathways, and provide at least 26 hours of contact time through coaching, education, and activities. The curriculum should be for 12 weeks and include nutrition and behavior modification topics. Two cohorts (each lasting 12 weeks) must be completed by June 29, 2027. Deliverables include Nutrition/Behavior Modification Curriculum (electronic), Program Set Up Manual (electronic), Physical Activity Curriculum (electronic), Training via Zoom (optional), and an Annual Fee.
