The contractor must properly plug each well according to the plugging plan (Attachment K) under Division oversight, using a service rig and work string. Wells 1500' or shallower require circulating cement to surface; deeper wells require multiple cement plugs. Contractor must extract tubing/rods, run tools to total depth, and cut casing 3 ft below surface. All tanks must be removed, flowlines cut and plugged, and sites reclaimed with permanent vegetative cover. TENORM scanning and disposal is required for contaminated material. Contractor must provide weekly progress reports and submit a Plugging Affidavit for each well and a Tank Facility Closure Affidavit for each tank.
