Kansas Highway Patrol (KHP)
Kansas Highway Patrol is seeking competitive bids for the purpose of supplying the TPR V MVE-1D form in accordance with the terms, conditions, and specifications.
The contractor must manufacture and deliver 80,000 four-part carbonless forms (8.5" x 11") with specific paper weights (Pink CB 15#, Yellow CFB 14#, White CFB 14#, Blue CF 15#), sequentially numbered from 0000001 to 0080000, with red ink for the seven-digit form number on Part 1 only. Forms are pre-collated into sets of 25, shrink-wrapped with chipboard back, packed 40 pads per carton (1,000 forms per carton), and delivered in one shipment to Kansas Highway Patrol Central Supply at 2019 E. Iron Ave., Salina, KS 67401. No overruns or underruns are permitted, and proofs must be submitted within ten calendar days after receipt of order.
