The Utah Army National Guard is issuing a Request for Quotes (RFQ) from qualified commercial vendors for the procurement of “Six (6) Person Audio-Booth”. All mandatory technical characteristics, specifications, and applicable terms are explained in the attached Standard Form (SF) 1449 and attached SPECs 6 Man Audiobooth. Prospective offerors are solely responsible for downloading, reviewing, and understanding all attached documents and requirements associated with this solicitation prior to submitting a quote.
The Contractor shall provide all necessary labor, equipment, and materials to install and integrate a 6-Station Single Wall Audiometric Booth, primary and expansion audiometers, and a bio-acoustic simulator. The Contractor is also responsible for ambient noise certification and establishing daily calibration references. Specific deliverables include a CL-M6 PR 6 Station Single Wall Audiometric Booth with an EAO-D-004 Add-On Door, a CCA-200 Mini Audiometer with accessories, a Benson Medical CCA-200mini Expansion Audiometer, and a BAS-200 Bio-Acoustic Simulator. The contractor must assemble and install the booth on site, install all audiometers and simulator into the completed booth, set and verify the daily calibration reference, and perform and provide SR-Ambient Noise Certification.
