The Navajo Area Indian Health Services, Gallup Service Unit – Tohatchi Health Center, Tohatchi, New Mexico, USA, is requesting for offerors to provide pharmaceutical medication returns, which base plus 3 option years.
This procurment is conducted under RFO Part 12 Acquisition of Commercial Products and Commercial Services. Where as the Government reserves the right to issue a firm-fixed price single award from this solicitation to the responsible offeror whose offer conforming to the solicitation will be the most advantageous to the Government, price and other factors considered.
CONTENT OF PROPOSAL SUBMISSION: All offerors will submit the following documents to considered for award on or by the proposal submission deadline:

The contractor shall provide pharmaceutical waste management services including annual characterization of the facility's pharmaceutical formulary to identify hazardous and non-hazardous waste streams; provision of compliant pharmaceutical waste containers supporting proper segregation (single-container approach for most hazardous/non-hazardous, separate containers for incompatible streams, and separate containers for trace chemotherapy waste managed as regulated medical waste); access to web-based pharmaceutical waste management and DOT training resources for facility staff; transportation and disposal of pharmaceutical waste at appropriately permitted RCRA-permitted facilities; and provision of documentation of transport and disposal via hazardous waste manifest or equivalent. The contractor must ensure replacement containers are provided and full containers are transported for disposal within 5 to 10 business days of notification. The contractor is responsible for all labor, materials, equipment, supervision, transportation, training access, and management necessary to achieve the performance outcomes.
