3.4 Period of Performance
This contract is expected to start upon Notice to Proceed (NTP). It is expected that the Contractor will work on administrative submittals and long-lead-time items during the initial part of the NTP period. No work, including administrative work, shall begin prior to NTP.
3.5 Cable Labeling
Labeling and identification of equipment, components, and telecommunications cabling shall be based on ANSI/TIA-606-C, Administration Standard for Telecommunications Infrastructure, and NFPA 70, National Electrical Code, as modified by the VA Infrastructure Standard for Telecommunications Spaces.
All passive infrastructure equipment, components, and telecommunications cabling installed shall be labeled at the time of installation.
Telecommunications cable labeling shall include delivery of complete and accurate connection documentation (identifying each cable's termination points at both ends), matching the installed labels.
Label locations shall be as specified in the VA Infrastructure Standard for Telecommunications Spaces, Sections 4.4.4 and related sections.
3.6 VA Information and Information System Security and Privacy Requirements
Recommended for inclusion (physical access baseline):
a. Contractor personnel assigned to work under this contract must undergo a background investigation commensurate with the risk and sensitivity level designation associated with the work to be performed, in accordance with VA Directive/Handbook 0710, Personnel Security and Suitability Program.
b. A Contractor/subcontractor shall request physical access to VA facilities for its employees and subcontractors only to the extent necessary to perform the services specified in this solicitation.
c. Contractors and subcontractors shall sign the VA Information Security Rule of Behavior (ROB) before access is granted, and shall complete VA Privacy and Information Security Awareness training (TMS #10176) initially and annually thereafter, if the CO determines any logical access to VA information or information systems is required.
d. The Contractor shall notify the COR/CO in writing within 24 hours of Contractor/subcontractor personnel separation, termination, or other events affecting continued need for access, and shall immediately take necessary measures to revoke facility access (including return of PIV/identification cards) upon contract completion, termination, or personnel separation.
e. The Contractor shall immediately (within one hour) report suspected security/privacy incidents to the VA OIT Enterprise Service Desk (ESD) at (855) 673-4357 (TTY: 711) and provide the COR/CO the resulting incident number within one hour of the report.

WAGE DETERMINATION
4.1 Service Contract Labor Standards (SCA) Wage Determination

To the extent this procurement, or a severable portion of it, is for services performed in the United States through the use of service employees and is covered by RFO FAR Subpart 22.10 (Service Contract Labor Standards, 41 U.S.C. chapter 67), the applicable Department of Labor Wage Determination for the West Haven, CT locality and labor category(ies) involved must be obtained from https://sam.gov/ (Wage Determinations), attached in full to this solicitation, and referenced in the SCA clauses below.
INSTRUCTIONS TO OFFERORS AND EVALUATION RFO FAR 52.212-1 AND 52.212-2
5.1 FAR 52.212-1, Instructions to Offerors Commercial Products and Commercial Services (DEVIATION AUG 2025) (Addendum)
Provisions incorporated by reference (by citation number, title, and date) have the same force and effect as if given in full text. Upon request, the Contracting Officer will make full text available. This version of FAR 52.212-1 is tailored for Simplified Acquisition Procedures and supersedes the standard FAR 52.212-1 text for purposes of this solicitation.
(a) Submission of quotations. Submit signed and dated quotations to the office specified in this solicitation at or before the exact time specified. As a minimum, quotations shall include:
The solicitation number;
The name, address, and telephone number of the Quoter;
The Quoter's Unique Entity Identifier (UEI) and, if applicable, Electronic Funds Transfer (EFT) indicator;
Information necessary to evaluate the factors contained in Section 5.2 below;
Responses to provisions requiring Quoter completion of information, representations, and certifications (other than those collected via the System for Award Management (SAM));
A statement specifying the extent of agreement with all terms, conditions, and provisions included in this solicitation and any amendments; and
The Quoter's proposed pricing for each CLIN identified in Section 2.
(b) Period for acceptance of quotations. The Quoter agrees to hold prices firm for 60 calendar days from the date specified for receipt of quotations, unless another period is specified here:
(c) Late submissions, modifications, revisions, and withdrawals of quotations.
Quoters are responsible for submitting quotations and any modifications or revisions to the Government office designated in this solicitation by the time specified.
Any quotation, modification, or revision received after the time specified for receipt is "late" and will not be considered unless received before award and the Contracting Officer determines that accepting the late quotation would not unduly delay the acquisition. A late modification of an otherwise successful quotation that makes its terms more favorable to the Government will be considered at any time and may be accepted.
If an emergency or unanticipated event interrupts normal Government processes so that quotations cannot be received at the designated office by the exact time specified, and urgent Government requirements preclude amendment of the solicitation, the time specified will be deemed extended to the same time of day on the first work day on which normal Government processes resume.
Quoters may withdraw their quotations by written notice received at any time before award.
(d) Basis of award. The Government intends to evaluate quotations and issue an award without discussions with Quoters. Quoters should therefore submit their best terms in their initial quotation. The Government reserves the right to conduct exchanges with Quoters, if necessary, to resolve minor informalities or ambiguities, but such exchanges do not constitute discussions in the sense used under RFO FAR Part 15 and this solicitation is not being conducted under RFO FAR Part 15 procedures. The Government may reject any or all quotations if such action is in the public interest, may select other than the lowest-priced quotation, and may waive informalities and minor irregularities in quotations received.
(e) Debriefings. If a post-award debriefing is requested, the Government will disclose, if applicable: (1) the significant weak or deficient areas in the debriefed Quoter's quotation; (2) the overall evaluated price and technical rating of the successful Quoter and the debriefed Quoter, and past performance information on the debriefed Quoter; (3) the overall ranking of all Quoters, if developed; (4) a summary of the rationale for award; and (5) reasonable responses to relevant questions about whether the agency followed the procedures set forth in this solicitation and applicable regulations.

5.2 FAR 52.212-2, Evaluation Commercial Products and Commercial Services (DEVIATION AUG 2025) (Addendum)

Evaluation Approach. The Government will evaluate quotations using the comparative evaluation process described in RFO FAR Part 12, under which quotations will be compared to one another to determine which provides the best benefit to the Government. The Government reserves the right to consider a quotation other than the lowest price that provides additional benefit(s) to the Government. Quotations may exceed the minimum requirements of this solicitation, and the Government reserves the right but is not required to select a quotation offering benefits exceeding those minimum requirements. Each quotation must meet the minimum requirements of this solicitation. The Government is not requesting or accepting alternate quotations.
The evaluation will consider the following:
Technical or Quality: The quotation will be evaluated to the extent it meets and/or exceeds the Government's requirements as outlined in the Statement of Work at Section 3, based on the information requested in Section 5.1 above.
Past Performance: The past performance evaluation will assess the relative risk associated with a Quoter's likelihood of successfully fulfilling this solicitation's requirements, as indicated by the Quoter's record of past performance. This evaluation may be based on the Contracting Officer's knowledge of and previous experience with the supply or service being acquired; customer surveys and past performance questionnaire replies; the Contractor Performance Assessment Reporting System (CPARS) at http://www.cpars.gov/; or any other reasonable basis.
Price: The Government will evaluate price by adding the total of all line-item prices identified in Section 2. The Total Evaluated Price will be that sum. The Government's Independent Government Cost Estimate will be used as one basis for assessing price reasonableness, subject to reconciliation of the Profit-figure anomaly noted in the Drafter's Note.

5.3 Representations and Certifications
FAR 52.212-3 is RESERVED for purposes of this solicitation. SAM.gov registration is required to be complete at time of submission of quotation. Quoters must complete annual representations and certifications electronically via the System for Award Management website at https://sam.gov/.
5.4 Contract Terms and Conditions
FAR 52.212-4, Contract Terms and Conditions Commercial Products and Commercial Services (DEVIATION OCT 2025) applies to this acquisition. To facilitate award, all quotations must include one of the following statements:
