Pharmacy Compounding Area Certification, for the Madison VA medical Center, Wisconsin

The contractor shall provide all personnel, equipment, supplies, facilities, transportation, travel, tools, materials, supervision, and other items and non-personal services necessary to perform semi-annual pharmacy compounding area certifications and monthly environmental sampling. The work includes testing and inspecting the hospital's pharmacy compounding area to provide certification in accordance with USP 797, USP 800, and CAG-003-2022. Semi-annual certification inspections shall occur in September and March, and monthly environmental sampling shall occur each calendar month. Specific areas to be inspected include the Ante Room (C1142), IV Prep Room/Non-Hazardous Drug Buffer Room (C1142A), Chemo Prep Room/Hazardous Drug Buffer Room (C1142B), and associated equipment such as laminar airflow workstations and biological safety cabinets. The contractor must provide semi-annual certification reports within 15 calendar days and monthly environmental sampling results reports within 10 calendar days.
