The contractor shall provide full test kits, logistical support for patients, and reporting tools for clinicians. The test kits will be purchased and delivered directly to the purchasing site, not to patients. The service includes mail-in semen analysis kits validated for a minimum of 48 hours of room temperature stability, post-vasectomy testing kits validated for room temperature stability for up to 7 days, full clinic portal access for reviewing and uploading results to EHR, and replacement kits at no charge if test results are delayed or invalid or kits are expired. The contractor shall provide CLIA certified and FDA registered prepackaged mail-in kits, and all report data shall be HIPAA compliant. The contractor shall provide test results via a secure web portal with clinician access, and may also send results via fax at the site's election.
